I usually associate this more with the following definition from Merriam Webster: to become very upset or angry *He lost his head and said some things he regrets.*

That first example is not how this idiom is used

Yeah this one is incorrect. It means to become panicked. Not just irrational. I'd also suggest that your first example has cause and effect backwards. Personally, forgetting everything I'd studied would be a reason to lose my head.
